Family Supports
Our goal is to help older adults stay supported, connected, and engaged as they age while maintaining a high quality of life. Our team provides holistic in-home assessments to evaluate an older adult’s areas of strength and ability to care for themselves. We can work with you to develop an individualized plan that works for your family.
Care Management
We offer care management to older adults who require assistance to maintain their current living arrangement or prefer to seek safe alternatives. Services can include resource navigation and coordination, linkage and referral to community resources including home health care and senior living options, post-discharge treatment & follow-up, personal affairs assistance and advocacy.
Counseling
Older adults and their family members can often benefit from compassionate counseling to cope with life’s transitions and other issues that come with aging. Our therapists are licensed clinical social workers with experience working with adults of all ages.
Volunteer Services for Older Adults
Our volunteers support older adults by offering companionship as Friendly Visitors, providing rides to appointments and other errands, assembling and delivering gift bags for major holidays, and providing technology assistance.
Virtual Programming
We offer a variety of monthly virtual programming for those 50 years of age and older to combat isolation and foster social outlets. Develop ongoing relationships and learn new skills to support your physical and emotional well-being.
